For new clients who have spoken with Dr. Daniel and are scheduled for their first appointment, please download and complete the Client Information form, the Client Rights and Responsibilities Statement and the Credit Card Authorization/Billing Policies form. Please read and fill them out as thoroughly as possible before your first appointment. Not all clients may need the Authorization for Release and this may be completed as needed. The Notice of Privacy Practices HIPPA is for your information and reference only and does not need to be returned to Dr. Daniel. Thank you!
